Under the guidance of the Program Coordinator and Clinical Chief, you will develop, implement and evaluate treatment plans based on evidence-based, client-centered care to promote independence and safety for older adults.
You will collaborate with a multidisciplinary team, document patient progress, and contribute to quality improvement and staff education.
