You will assess patient needs, coordinate discharge efforts, and utilize your nursing experience to manage caseloads efficiently. Key Responsibilities:
• Provide advocacy for patients to optimize care
• Manage a diverse caseload, focusing on individual strengths
• Develop and coordinate discharge planning efforts
• Assess, refer, and implement interdisciplinary plans
• Adjust priorities based on a fast-paced environment
Requirements:
• Required RN license; ACM or CCM licensure preferred
• Associate’s degree in Nursing necessary; Bachelor’s preferred
• Minimum 2 years in an Acute Care setting required
• 2 years of experience as a Registered Nurse
• Strong capacity for organizational and leadership skills
