Flemingdon Health Centre (FHC) is launching a population health clinic and seeking a Lead Physician to provide comprehensive primary care within an interdisciplinary team. The role focuses on improving access, attachment to care, and health equity for communities in East Toronto.
You will lead clinical and systems design, engage partners, and contribute to population health goals while supporting staff and ensuring high-quality, patient-centred care in a community-based setting.
