Mental Health Care Coordinator

Support members through mental health programming and coordinated clinic care as a Clinical Care Coordinator at Amae Health. This hands-on role is in person.
As part of an interdisciplinary clinic team, you will support members with severe mental illness. You will facilitate activities, manage communications, update records, and assist scheduling. The work also includes sharing observations with psychiatrists, therapists, dietitians, and primary care providers.
Key Responsibilities:

Facilitate groups, arts, crafts, and recreational activities
Welcome members and manage phone and email communications
Assist scheduling and update member records accurately
Share observations during interdisciplinary team meetings
Connect members with community resources and supports
Requirements:

Enrollment or recent graduation in a related degree program
Interest in mental health and severe mental illness
Strong interpersonal skills and collaborative approach
Organization, attention to detail, and basic computer skills
Comfort working individually, in groups, and across changing situations
Bring empathy, organization, and member engagement skills to Amae Health’s interdisciplinary mental health care team.
