Community Registered Practical Nurse

Deliver community nursing care as a Registered Practical Nurse with autonomy, patient assessment, and care planning across scheduled home visits.
ParaMed is seeking a Registered Practical Nurse for part-time community care. Support patients of all ages and varying complexity, with Registered Nurse support for more complex patients. Help patients remain comfortable at home while developing therapeutic relationships and clinical skills.
Key Responsibilities:
Participate in scheduled community visits within your scope
Provide ethical and safe nursing care
Assess individual patient needs using appropriate techniques
Execute, monitor, and adjust care plans
Maintain professional patient records and encounter plans
Requirements:
Current CNO registration as a Registered Practical Nurse
Valid driver’s license and access to a vehicle
Reliability, flexibility, and willingness to travel
Experience with vulnerable populations
Required after-hours on-call coverage for patient safety
Bring community nursing, assessment, and care planning expertise to support ParaMed patients in their homes and communities.
