Community Care Manager Guelph

Use clinical assessment and relationship-building skills as ComForCare’s Community Care Manager in Guelph, with a hybrid 50/50 schedule.
This Community Care Manager connects clinical services with the wider community. The work combines in-home assessments, care planning, family onboarding, and outreach. You will build healthcare partnerships across Guelph, Cambridge, and Waterloo Region while guiding families through private care options.
Key Responsibilities:

Evaluate physical, cognitive, and social needs in clients’ homes
Draft comprehensive, person-centred care plans
Guide families through intake and warm introductions
Conduct reassessments and follow-up visits
Visit healthcare organizations and attend community events
Requirements:

Registered Practical Nurse or Registered Nurse in good standing
Registration with the College of Nurses of Ontario
Local healthcare or home care system experience
Relationship-driven communication with families and professionals
Valid G-class license and reliable vehicle
